If you prefer to complete the application yourself, below are instructions.
(1) Participants are encouraged to complete, save and submit the application electronically by email. Please
note - you may need to open the application in Adobe to complete it. Please access the 'OPEN WITH
DIFFERENT VIEWER' tab in upper right hand corner of application webpage to open in Adobe.
(2) If you're unable to open in Adobe, you may also print, complete, scan and send the document.
(3) Employer and/or Medical Professional certification may also be sent by email.
